A NEW £28million “Better Care Fund” will help improve health and social care for elderly and vulnerable people throughout Warrington.
The fund has been established by Warrington Borough Council and the NHS Warrington Clinical Commissioning Group, and will cover a range of health and social care related services that will reduce hospital and care home admissions, and will help people to be discharged smoothly and safely from hospital.
Chair of Warrington’s health and wellbeing board Professor Steven Broomhead, said: “By pooling our resources and joining-up various services we will be able to provide better support to vulnerable people. This will help them to carry on living independently in their own homes, where they are connected to their communities, avoiding or delaying the need for them to be taken into hospital or to move into care homes.
“In Warrington, the council and its health partners have worked for a number of years to create more joined-up services which help people to regain independence after a hospital stay and avoid emergency admissions to hospital. The Better Care Fund will help us to strengthen our joint approach.”
